For each Digital Voice line you have ordered, follow these steps to fill in the form below:
  1. the line by checking the checkbox next to the line number.

  2. Enter the phone number and password for the line. This information has been provided to you in your service confirmation notice.

  3. Choose a for the line.

  4. You may also select whether Caller ID for outgoing calls you make are or show your phone number.
